Operation Mend is a nationally unique program providing specialized surgical and psychological care for veterans, service members, and their caregivers. The Semel Institute of Neuroscience and Human Behavior is seeking to hire a full-time Case Manager to join our Operation Mend Department. Operation Mend is a nationally unique program providing specialized surgical and psychological care for veterans, service members, and their caregivers. You will provide comprehensive case management for patients seeking physical and psychological injuries. Primary duties include:
- Collaborate with multidisciplinary team to engage, educate, communicate, and coordinate care and resources for participants
- Assist with developing and monitoring treatment plans for all participants
- Maintain continuity of care, identify needs, review goals and implement action steps
- Complete required documentation and electronic data entry
- Assist in the preparation of reports
- Other duties as assigned
Job Qualifications
Qualifications
- Advanced degree in a behavioral health field (MSW, MFT, PhD, PsyD)
- Valid CA license in behavioral health field
- Experience working with trauma, veterans, and caregivers
- Experience working with families from diverse backgrounds
- Excellent written and verbal communication
- Ability to work effectively and closely with other team members
- Skilled in management of time and setting priorities
- Proficiency in Microsoft Office Systems (Word, Outlook, Excel, and Access)
- Ability to work flexible hours
