Job Title: Case Manager


Job Description

As a Case Manager, you will be responsible for communication and relationship building with clients and all collateral partners, including schools, funding partners, therapists, agencies, clinicians, supervisors, and foster homes. You will set limits for youth, develop interventions, and set goals toward individual successes in conjunction with clients' guardians, therapists, and other providers. You will initiate and monitor external recommendation processes and treatment planning for clients in your caseload, ensuring compliance with policies and procedures. You will interact in a timely and effective manner for computer reports, tracking, and paper reports according to agency protocols. Participation in on-call services, representation on task forces and community boards, and attendance at meetings are also required. Strong interpersonal skills, discretion, and confidentiality are essential.


Responsibilities

  • Build and maintain relationships with clients and collateral partners such as schools, funding partners, therapists, agencies, clinicians, supervisors, and foster homes.
  • Set limits for youth and develop interventions and goals toward individual success in collaboration with clients' guardians, therapists, and other providers.
  • Initiate and monitor external recommendation processes and treatment planning for clients.
  • Ensure compliance with policies and procedures, including all contract sources.
  • Timely and effective reporting for computer reports, tracking, and paper reports in accordance with agency protocols.
  • Participate in on-call services as needed.
  • Represent the agency on task forces and community boards regarding FBS interests.
  • Exercise strong interpersonal skills throughout all levels of the agency.
  • Attend meetings throughout the agency and with collateral partners.
  • Stay updated on trends and significant events in the field of Human Services and Case Management.
  • Analyze situations, identify problems, and evaluate alternative courses of action.
  • Plan and organize work effectively and ensure its completion.
  • Use discretion and individual judgment with agency guidance and direction from the supervisor.
  • Maintain the highest level of confidentiality.
  • Accurately and completely document all work and share as appropriate within the agency and with collateral partners.
  • Perform other duties as assigned related to primary responsibilities.

Additional Skills & Qualifications

  • Bachelor's Degree required
  • Valid driver's license
  • Verification of adequate personal auto insurance on the vehicle(s) to be used, including minimum coverage limits of $100,000 per person/$300,000 per accident for Optional Bodily Injury to Others and at least $100,000 for Damage to Someone Else's Property

Work Environment

The work environment includes a typical office setting with common areas and offices, where the noise level is generally low to moderate. The setting also includes individual homes with environments unknown to the agency. The schedule is Monday through Friday from 9am to 5pm, with potential for extended hours or weekend work if needed.
