MTM Pharmacist

Description

• Perform comprehensive medication therapy reviews with eligible members; review member’s medication profile during session with member to identify, prevent, and resolve medication-related problems to help members maximize benefits and minimize cost; document the member encounter in the MTM platform and complete outreach to other health care professionals as necessary

• Conduct encounter with members to improve outcomes for member and for clients related to topics of medication adherence, gaps in therapy, and other measures; document encounter in platform and complete outreach to other health care professionals as necessary

• Contribute to projects as needed to support business needs

• Collaborate with team members in MTM/CCM support roles by providing assistance and guidance as needed

• Other duties as assigned

Additional Skills & Qualifications

Qualifications/years of experience:

• Doctor of Pharmacy (PharmD) or Bachelor of Science Pharmacy Degree

• Current pharmacist license in good standing with the State Board of Pharmacy in the State where the position is located

• Must be eligible to work in the United States without need for work visa or residency sponsorship

• Prior MTM/ambulatory care experience preferred

• 1 year of experience as licensed pharmacist

*Preference is for retail/insurance/PBM pharmacists and not hospital/clinical pharmacists (Hospital environment is more black and white, whereas working at Prime requires some working in the gray with the information available)

Must Haves:

1. Communication skills are top priority as direct patient interactions are primary function. Needs to build rapport quickly with patient and show empathy over the phone.

2. Ability to collaborate and support leadership initiatives within the larger overall team

3. Self-directed and able to reprioritize quickly in a changing environment

Technology Requirements:

• Proficiency in MS Office products
