The Care Management Nurse, MDS Nurse works the RAI process and conducts assessments and care plan coordination for those residents assigned. Some responsibilities of the MDS nurse include:

<li class="MsoNormal" style="text-indent: -0.1pt;">Attending care conferences for sub-acute residents/guests</li>
<li class="MsoNormal" style="text-indent: -0.1pt;">Attending Medicare Meetings</li>
<li class="MsoNormal" style="text-indent: -0.1pt;">Assist in procuring any item related to the discharge process for residents/guests anticipated to be returning to the community.</li>

<li class="MsoNormal" style="text-indent: -0.1pt;">Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community. </li>
