Reporting to the Director of Nursing, the Admission Nurse ensures the best practices in nursing care are followed and that they meet the standards and mandates of long term care required by state regulations. The ideal candidate holds a current nursing license in this state, has graduated from an accredited Registered Nurse program, and has three or more years of experience, preferably in the long term care arena.</div>

<li>Supervises nursing staff, including orientation of new staff members, ensuring that the quality of care, the quality of resident life, and the quality of staff are of the highest standards</li>
<li>Assumes responsibility for the safety and welfare of residents in the absence of the DON, and ADON</li>
<li>Helps Charge Nurses, as needed, with Physician visits, decision making, directing CNA work assignments, treatments, monthly summaries, admissions/discharges and transfers, and order transcription</li>
<li>Assumes responsibility of Charge Nurse Duties in his/her absence</li>
<li>Maintains open communication with residents, families, volunteers, staff, and other stakeholders</li>
<li>Participates in meetings, in-service trainings, and staff development events</li>
<li>Monitors resident care, documentation, and Health Care Center environment for compliance with appropriate state regulations</li>
<li>Assesses employee and resident injuries, renders first aid or makes decisions for appropriate referral to MD and/or ER, and adds situations to 24 hour report and forwards incident report to Nurse Manager and/or DON</li>
<li>Helps the Nurse Manager with Care Plans</li>
<li>Coordinates replacement staff for call-ins </li>
<li>Coordinates transfer of residents to hospital in the event of a medical emergency</li>
<li>Provides input regarding residents scheduled for Care Conferences</li>
<li>Provides leadership, guidance, supervision, and evaluation for the nursing staff</li>
<li>Demonstrates best practices for staff to follow</li>
<li>Provides care following all federal, state, and local regulations</li>
<li>Rotating On Call schedule is required</li>
<li>Performs other duties as assigned</li>

The Care Management Nurse, MDS Nurse works the RAI process and conducts assessments and care plan coordination for those residents assigned. Some responsibilities of the MDS nurse include:

<li>Assesses resident through physical assessment, interview and chart review.</li>
<li>Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.</li>
<li>Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.</li>

<li>Coordinates, identifies, and/or initiates significant change MDS’</li>
<li>Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator</li>

<li>Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.</li>

Qualifications:

<li>Registered Nurse, RN or Licensed Practical Nurse</li>
<li>AANC certification a plus. RAC-CT</li>
<li>Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.</li>
